Specialties differ a lot in how open they are to IMGs. In some, nearly half of all residents are international graduates; in others, very few programs take IMGs at all. The median IMG share on this page is the quickest way to see where your application is most likely to be read.
Visa sponsorship matters just as much if you need a J-1 or H-1B: a program that does not sponsor your visa cannot rank you, however strong your application is. Inside each specialty you can filter programs by visa and by IMG share, and each program page lists its Step 2 CK expectations, graduation-year limit and interview rates.
By the share of international medical graduates among current residents, the most IMG-friendly specialties are Internal Medicine (median 46.5%), Pathology-Anatomic and Clinical (median 40%), Family Medicine (median 22%), Neurology (median 20%) and Pediatrics (median 20%). Within each specialty the share varies a lot between programs, so compare programs individually as well.
You can apply to as many specialties as you like through ERAS, but each needs its own personal statement and letters that fit it. Most IMGs focus on one main specialty and sometimes add a second, less competitive one as a backup.
